Sadly no they are closed to the public due to COVID ... went there this a.m for a replacement birth certificate. The officer/ concierge pointed me to a stand with all the apps for different certificates explaining I'd have to get it using mail in , phone or online. Good luck

There's no form. You need to come to the Vital Records office (legal department) and tell them what exactly you need to amend. Some things they can amend right there, for others you'd need to get a family court order first. In any case make sure you bring all the supporting documentation with you.
