I think the best option is to do what I did to avoid unnecessary processing fees. Pay the 150$ to the state with a debit card and 100$ to the clinic cash. Once the 150$ state charge hits your bank account you then immeditely know your precious card is LN THE WAY!

Correct it’s $249 every time you need to renew . The state charges $150 every time you need to renew and we charge $99. Great news is the card is good for 2 full years instead of one now. So basically half price with that being said alone. We are by far the lowest cost !! You certainly won’t find a better group of people who care and are actually patients ourselves than at Marijuana Evaluations. We have a way lower cost than any others. Unfortunately there are people out there like Angela Maldonado that are only here to troll n spread lies. I happen to know for a fact the place she refers charges way more than us. And an hour is way too long we get people in and out in 20 minutes or less. N we do give you a few hundred dollars worth of dispensary coupons. Come see the best at Marijuana Evaluations . You will need a qualified complaint. Look up what they are. My daughter had shoulder surgery 15 years ago, no previous medical records. The doc manipulated her shoulder and she was given her card. The doctor there gave such great advice that works for me. Good luck love.
Yes you can and they are great. For the best perfect outcome bring your $249 in cash. You come in fill out paperwork and then you have a seat and wait for doctor. If you have anything that states a medical condition bring it not necessary because you see a doctor there. Then it can take 10-14 business days to get your card if you are approved. 😁
Yes but there is a small 6$ transaction fee so we recommend bringing the $99.00 certification fee in cash so we don’t see that charge :) the state does need their fees on a card (no extra fees)...great question :D
I got mine within 12 days if your moving I would call and let them kno my friends was sent to wrong address last time [thru a different doctor] and they sent it back and she had to wait another 2 weeks before she got hers.

After you fill out the proper paperwork it can take anywhere from up to an hour to get in. Then once your accepted it can take 7 to 10 days for them to email you your new card.

If you're on food stamps your looking around 150.00 dollars, and for that you would need either your award letter or your food stamp card with both it has to have your name on it. If you're not on food stamps than your looking about 270.00 dollars. You also will need a current I.D. and an envelope that has your current name and address. They're very efficient they're a great place to go.
U dont need ur records for this place just ur food stamp card with your name on it or approval letter from des and ur driver license and it comes to 190.00 with ebt discount with the 8 dollar processing fee if u use debit which I did or it wouldn't be 182.00 cash I believe
FYI MMJ EVALUATIONS IS GARBAGE. GOOD LUCK ON GETTING THAT CARD!! IT TOOK THEM 9 DAYS TO FINALLY SUBMIT MINE AND 16 TO SUBMIT MY SONS DADS, MIND YOU THEY CONFIRMED THAT HIS HAD BEEN SUBMITTED ON THE 9TH AS WELL. WHICH BOTH CARDS SHOULD HAVE BEEN SUBMITTED ON THE FIRST WHEN WE APPLIED.
If State hasn't taken the $150 yet, then your name won't show yet in system till Application is Approved & Processed by AZDHS. Once they do that and you recieve an Email from AZDHS stating your Application was Approved. You can then go to their website and access your Online Digital Medical Card from there. Takes 5-7 Business Days [Wknds don't count] before they recieve and Approve your App
Mmj Cards are now Digital, so once you leave Clinic and get Approved for Card. You'll recieve a Email from AZDHS saying you're Approved and they'll take the $150 from your Debit Card on File and you'll access your New Digital MMJ Card on www.AZDHS.gov within 24-72 hrs from Approval

Once they send you your Approval Email, you can go to their website azdhs.com and set up your Profile by using your Email they have as your "Username" and Select "Forgot Password" below to set up a New Password and then you'll be able to access your Digital Medical Card Online

It varies different doctors through out the week. But pretty easy to obtain a medical card without compcation.
In order to get a medical marijuana card in the state of Arizona you do you have to have either an Arizona ID or a US passport. We can get you certified without those and you can send them by email later but keep in mind the actual card you won’t receive until we get one of those forms of ID. The total cost of the door normally is $249. The way that breaks down is $99 of that goes to the doctors office and $150 going to the state of Arizona. However if you receive food stamps in the state of Arizona, then the state fee is lowered to $75 instead of the $150. So if you do receive food stamps the total price out the door would then be $174. Unfortunately no. In the state of Arizona major illness and pain are what qualify for a card. Back pain for instance.
Up to two weeks to get the card and you can go to the dispensary on the same day you get it.
