They really wanted to schedule me but didn’t give me enough information and ended up wasting my time. I Called was told to bring 3 things: records, money and ID. I brought all of these but they needed a doctors note within a year. This would be fine but 2 other patients were there having similar issues (the staff not giving enough information). I will not be back.

My card renewal didn't go smoothly I waited 2 weeks because they didn't put in the correct email after I ask multiple times to confirm the email was correct there mistake when I called the office they said Call AZHDS not there problem they'll make mistake then be rude customer service you get what you pay for

'Bout $250, maybe 255. That includes the state fee. If you receive snap benefits, aka EBT, aka food stamps, you qualify for a state fee discount and then the total cost will be about 175. The card is good for one year from issuance before you have to renew, and I believe you can complete the renewal process up to 3 months before your card expires and your next card will be good for another year beginning the day after the first card expires. If you wait until your card has expired to renew, you'll have to repeat the entire process again (which is still one-stop at this location), and the year for that card will begin later (a week or two after you submit the application, or a couple days before you actually receive it in the mail. It's worth every penny to avoid prosecution for marijuana, and you'll get killer first-time patient specials at almost all dispensaries. Most also run great sales very often. The card will pay itself off quickly. Hope this helps!

Walk ins are based upon availability, appointments are preferred.
They usually request what are called Chart Notes, Progress Notes, or SOAP Notes signed by a doctor. Make sure it's not a summary!
Not in AZ. The state department must first issue you a card so they can keep track of purchases.
