I got mine within 12 days if your moving I would call and let them kno my friends was sent to wrong address last time [thru a different doctor] and they sent it back and she had to wait another 2 weeks before she got hers.
You will get an email from az department of health services. Email will tell you if your accepted or not if you are you will get your card no more then 5 days after receiving email
