If you're replacing a lost ID, then bring in your birth certificate (a passport is acceptable if you have one) and proof of your address. Proof of address can be a recent bank statement or paycheck stub with your name and address. If you don't have that, and you live with an adult, the adult can sign a recent utility bill or bank statement and write on it that "(yourname) lives with me at this address."
