I assume you want a regular Missouri state ID for your 15 yr old, not a Drivers Permit or Real ID. If so, he’ll need his certified Birth Certificate, Social Security Card, and proof of residency. A recent document from his Missouri school with his name and address or a recent bank statement would suffice. If he doesn’t have such a document, then you can bring a recent utility bill, bank statement, paycheck stub, etc., in your name and address, and you can vouch that he lived with you at that address. A parent must accompany since he's a minor.
