If you want to know if you're listed as a beneficiary, then provide a letter by mail or in person simply stating your name, contact information and whether you are listed as a beneficiary. Also include the name and pension number of the member. If you have not done so already, report the passing away at NYCERS.org and provide the original death certificate asap to get things going
