Go the website, there's a form to download. Make sure you chose the right form, renewal verses application. The processing you want, that's either expedited or normal processing. It has instructions on the number of passport required. The side of the passport will be the same as what is used at the American embassy in Ghana or here in America but they are not picky about that. Hope this helps
