I have worked for a Dr office over 10 years , full time. I worked 206 hours in one month. I mentioned it to my employer that i notice I never get paid overtime. He said I was starting trouble and that he don’t pay for overtime. What should I do . ?

You will need to file a wage claim for the overtime he owes you. If he takes any retaliatory steps, you can also file a retaliation claim.
