Diana pretty much sums it up. Along with a birth certificate you will need a government issued form of identification. i.e. Driver's license, ID Card or ??? Good luck.

You can but depending on the cob officer they will probably still lecture you about needing the real one and/or a passport. And if the copy is legible and not all torn up from being folded up so much it will be better. And Also you are not supposed to laminate them to keep them from getting all torn up. But again, they will most likely let you use it but may lecture you on it depending on the officer and their mood. Sadly.
