Download the form MV82, fill it out and mail it in with the payment. According to the DMV the fee for a title only is $50. Send in a money order with the paperwork. Madison County DMV does have a drop box in the parking lot to save having to wait for the mail to deliver it.

Just know that the new title does have to be mailed from Albany, they do not print them on site in Wampsville. But it shouldn't take only a week or two to get it.
