I do not have a copy of my birth certificate. I do have a work ID, SS card, W-2 form, & a utility bill. Would I be able to replace my NY state ID?

If you have your previous ID then yes if not then no. When you go to the DMV floor their going to give you a requirement paper that should tell what are the other options you can bring in for you to get a replacement for your ID.

You should be able to. As long as it has your current address. You can also bring your passport as well.
