You guys can always contact governor ducey office for assistance in getting your manners handle when this state agency isn't doing much of anything to help. Not sure if senate does anything but you can contact Senate sinema office they are usually good at responding
Aaron Birney-krick
Alot of those problems that other people put down aren't all there I think cause there are apps to block a call and such but.. you guys have horrible customer service aren't organized communicating wise or file. and takes a very long time to connect to anyone there. Sorry you guys have the same treatment of a greyhound station.
Lucy GL
They are CONSTANTLY calling and harassing, even when asked to be taken off the call list. Causing unnecessary stress during my pregnancy. STOP CALLING AND LEAVING VOICEMAILS!!!!
Kacey Miller
Everyone I've come into contact with here has been extremely helpful and kind. People helping people; its the best!

I have been trying to find out as well. Some one who knows please answer.
That could be any hotel in all of AZ
how do i get copy of covid vaccination i lost it
Hi Robert - I suspect that you should be able to get the vax when your husband does. My 93 yr old mother had her appt for the Pfizer shot last week at the State Farm Stadium in Glendale. I am her sole caregiver, so they allowed me to register and get my shot at that time, too.
This is true in many cases....Shame
This is an administrative building with no medical services to the public. You can go to azdhs.gov under immunization and give them a call and they can direct you where you can go.
Ask your provider for better advice
I have same question, only for Goodyear.
Notify the patients families. You can't trust the government to move quickly or do the right thing
Hi Sharon, Good Question. For those that don't know. Most states offer an Aging Assistance Program. They provide multiple levels of help to seniors of many different situations. The AZ Office website is: https://www.aaaphx.org/ You and anyone over the age of 55 can find help there. Google created this question and answers section so you can ask and receive help from those that can. That's why we have different opinions levels and review status ratings. You're question was very worthy .
No Shoes No Shirt No Mask No Vaccination Record No Service
The online website
Sorry I don't work for that department. I don't know why it would be on the website at all. Too hard to do I would think.
NOO
Go to the provider that gave you a card and ask for a replacement.
We don't do that
No. You should have received that at the time you got your shot.
What is approved and available.
It depends on what is available for at that location. They both do the same thing.
It is on the webpage for ADHS and the counties environmental health services home page woulld have the updates on when they would have appointments.
Yes, usually a couple days prior and the day of. You also receive a covid card with dates of your first vaccine so on the 2nd vaccine the card will be dated and marked you received full vaccination.
Unsure.
Not sure how they are doing it im not longer at DHS
You would probably have to contact the health department directly
Don't worry about it. Stay where your at if you're scared
Get hired by a caregiver company and get your mother on their list of people who need care.
CDC guidelines are Lowest risk: Virtual-only activities, events, and gatherings. More risk: Smaller outdoor and in-person gatherings in which individuals from different households remain spaced at least 6 feet apart, wear masks, do not share objects, and come from the same local area (e.g., community, town, city, or county). Higher risk: Medium-sized in-person gatherings that are adapted to allow individuals to remain spaced at least 6 feet apart and with attendees coming from outside the local area. Highest risk: Large in-person gatherings where it is difficult for individuals to remain spaced at least 6 feet apart and attendees travel from outside the local
They bring on their own deaths
They have birth records for people born in Arizona, yes.
Does it matter? Do what your boss tells you to do!
Dept of Vital Records. Maricopa https://www.maricopa.gov/2770/Vital-Records
Candida auris is a species of fungus first described in 2009, which grows as yeast. It is one of the few species of the genus Candida which cause candidiasis in humans. Often, candidiasis is acquired in hospitals by patients with weakened immune systems. Find out more at https://cdc.gov This is the center for disease control. Should help.
Most acute episodes of diarrhea are due to viral infections and last three to five days. Prolonged diarrhea -- lasting more than four to six weeks -- usually is due to an underlying gastrointestinal disease, the most common of which are diseases that cause inflammation and malabsorption of food
No
It sets a minimum CNA-to-patient ratio of 1:20. Licensed nurses must provide a minimum of one hour a day in direct service to residents, and a facility must have at least one licensed nurse for every 40 residents
You can get proof a number of ways. Including the administer of the vaccine. Even old school records will have this if you're an adult. The second vaccination must be at least 28 days after the first MMR vaccination. Your record must include the month, day, and year of yourvaccinations. One of the twoMMR's must have an administration date after 12/31/79. International students must also provideproof of 1 Rubellavaccination.
No, you will need to obtain your card from a certified MMJ physician. Can be found online. Have medical records ready
First step is to file a written complaint with the facility. Send cert mail if needed. I understand you may feel upset and like it may ne an internal problem, but I recommend trying the chain as well as filing a report. Here's a link to the AZ Board of Health Examiners, should help: https://www.azbbhe.us/ Make your case and be proactive if you can, go to rehab meetings and stay involved. Sometimes the best medicine is having someone like you who cares... Good Luck
You can rescind , but the gun law is Federal. Once the choice is made to obtain the card and go against Fed acts and laws. Some rights are given up. Anyone with a state issued MMJ card is not allowed to own or carry a firearm.
I would ask a doctor at an urgent care or er instead of waiting till they open tomorrow morning. Those kinds of issues are pretty time sensitive
Arizona medical board can help with that. Adhs doesnt handle issues with doctors
The county health department handles those issues
Yes. It's $10 and you get it from same place that issues them. The dept is health and human services
Govenor Ducey's office handles those complaints. Arizona Department of Economic Security. There is link you can email the Govenors office specific to complaints. They respond with 3 days maX.
Department of Economic Security health services. Or go on tbeir website .However, since your friend is an adult, I assume, due to privacy laws and her rights, she has to agree with the decision. You would have to have a power if attorney or notarized letter of her allowing. You to make decisions for her.
This is usually administered by an RN at the facility of employment. Otherwise Concentra or Walgreens / CVS
150 if your not on food stamps 75 if you are.
Call the police (non emergency) line to request a welfare check. The department of health does not perform these types of services
To be honest I'm not sure...sorry
Definitely call the dept of health and human services. It may have gotten lost in the mail. Mine got sent to the wrong address and was returned to them the first card I got. Unfortunately that happens once in a while.
Look in article 1 the rules are in there now
Just flu like everywhere else
Not to my knowledge, but you might look at the temp requirements
How do I report and file comlaint against mental health case management team for HIPPA violation and neglet of care on Copa Health, Marc Center and Partners in Recovery Metro Campus
I am sorry. What is your question?
This is significantly higher than safe levels (10ppm) and has been a problem that has been addressed with the water company by authorities in the past but appears nothing is being done. It is NOT something the water Company or city are notifying residents of and this level can kill people
Call your local pharmacy, they should be able to direct you to where they can be turned in. Hope that this helps.
Yes
Depends in what capacity...?
