This is a little late but I hope this helps someone else, that being said I suggest asking the MVC directly for clarification, according to NJ motorcycle manual anything above 50cc is considered a "Motorcycle" so for a 200cc scooter you would need to get a motorcycle license or endorsement which the dmv does provide the related services at this location. Generally they don't give you scooter or motorcycle to test on you would have to provide your own or borrow one. That being said I would suggest looking into taking a basic riders course which is a motorcycle riders training course that you do have pay for but they provide you a motorcycle and helmet and you can get your license/endorsement that way. For your final question the prices are listed on the NJ mvc website

Their is more then 1 for license but they are further away cause its only the regional locations that do restorations and licenses. This one is very efficient I was in and out in under an hr with no appointment I just walked in but you have to be there by 8am cause it fills up fast

You have to get a CDL permit 1st, that's $125. It's a Class A with all of the endorsements, Hazmat, Tanker, Doubles/Triples, Passenger and School Bus. You would then take test to narrow it down to what you want, Class A, B or C and the endorsements you need.
