Something is amiss at this company and somehow this has slid under the radar. Thousands of layoffs across the country in the last year or so. This company is also outsourcing many of these positions over sees. There seem to be more executives, managers, supervisors, leads and team leaders than actual team members. Regence has failed to land any new large contracts in recent years including Amazon and Nordstrom. Even though they wasted tens of thousands of dollars staging their offices with modern furniture and decor. It is rumored that Regence hired temporary employees to stage in their offices as to make them look busy to prospective new clients touring these offices. In closing, if I were a current employee of Regence, I would be getting my resume ready and I would update my references as you may be getting laid off tomorrow.

Depends on your plan. Some plans require a referral from primary physician, while others don't. Also keep in mind many specialists, labs, and radiology depts require some type of referral(even though your insurance plan may not). I recommend you contact your health plan customer service center to speak with someone who can verify this. Hope this helps.
It will most likely have multiple claims, for facility, surgeon, anesthesiologist, etc. And as said priorly: copay sometime apply to an office visit but that is plan depending...
