Hi Richard, We usually have specials going on that would put the total cost at $280. This includes a visit with our physician, a physical exam, and the Arizona application fee. As a courtesy, we submit our patients' applications for free. We are always happy to tell you the exact rate for your visit if you give us a call. Hi Richard. For Renewal Patients who have not seen a physician in the past 12 months, we are able to perform limited in scope physical exams. The total price including that exam ends up being $280. That includes Arizona's Application fee of $150 (reduced to $75 for SNAP recipients), our Office Visit Fee which is on special right now for $100, and our Physical Exam Fee of $30. I renewed for $265 total. Not sure if I got a discount because I'm a veteran or because I used to Greenleaf again. All prices and info is on their website.
