My regular MMJ place to get my card renewed closed down, I had to look for another place. I tried a facility by me and the people were so rude to me that I just left. I asked the people at a dispensary I go to about where I can go to get my card renewed. They gave me a flier with your number on it. It was easy to make an appointment. The location was near to me. The people here were awesome! I gave them my information. It was easy filling out forms through email. I have nothing but good to say about my experience!

Hello. Your wife should bring in medical records from the physician who is treating her. The records should be from a visit within the past 12 months and include the chart notes.
