Hello, Our office visit is currently on special for $115 if you are a new patient and $100 if you are a renewal patient. Arizona also charges $150 for an application fee (which they reduce for SNAP recipients). In certain cases, physical exams are available for an additional $30 fee. Typically, new patients pay a total of $265.
