Hello. Your wife should bring in medical records from the physician who is treating her. The records should be from a visit within the past 12 months and include the chart notes. She will also need a valid ID and should be ready to have a photo taken. Give us a call and we will be happy to answer any questions you have.
They will need your wife's medical records with all most recent diagnosis and approx. $250 for the license and Dr visit.
