Go down town to Probate court.
Ms. Calhoun Probate Court retains a true copy of the document and is the custodian of all the records. However, if you are just starting the process of a name change you may download blank forms from the Probate Court Website (www.probatect.org/forms/name-change). Click on CHANGE OF NAME OF AN ADULT as well as the INFORMATIONAL PAMPHLET. Download both documents and follow the instructions. -------------------------------------------------------------------------------------------- If you’ve already gone through the process of the name change and request a copy of the JUDGEMENT ENTRY, please visit the Hamilton County Probate Court 230 East Ninth Street, 9th Floor.
