A court order will terminate a guardianship upon the death of a ward, upon the ward being adjudged competent, and in the case of a minor, upon the ward reaching the age of majority (18). If the ward moves to another county within Ohio, the guardian- ship may be transferred. You may download a PDF application to terminate a Guardianship. Go to: www.probatect.org. Select FORMS and then Guardianship Probate Court is open Monday through Friday from 8:00am to 4:00pm. If you or your attorney needs help with the form please call 513-946-3600. That depends on a lot of factors. If you live in Hamilton County, though, this would be the place to start. The staff is usually very helpful if you have questions and no attorney.
