It depends on your child's age. 5 and under you can use birth certificate and official shot records. But if the child is between the ages of 6-17 you will need school transcripts, or a doctor exam record, if the child has a school id that is acceptable as well. And you will need to provide your ID too..
Birth certificate, your license and to be safe I would bring a piece of mail with your name and address. To be sure I would call them.
