I have recently obtained my records, not Woodlawn, but hospitals. It IS YOUR chart so you do have a right to obtain it. There might be a small fee, you need at least 2 forms of ID, one with a photo. Also, It depends on how long it has been since you were there and their policy. I was able to get mine some places, some not. I #2was at 1 hospital with f/u in their hospital physician from 2005-2010. My records had been destroyed. However, a different hospital provided my chart on a CD easily, no fees. You need to call Woodland, select "Medical Records," or ask the front desk if answers. Good luck!
