NYCERS does not issue W2 forms, you get that from your employer. NYCERS issues 1099r forms. You may download the 1099r by visiting MyNYCERS.org and signing into your profile. If you cannot sing in or forgot your security reset (i.e 'forgot username/password links), then call and ask for a reset of your online profile.
