I need to renew an old passport that expired on Feb 24th 2015. I have passport, drivers license, and social security card. Do I need to come in to Passport Agency in person? Also, do I need any form of a birth certificate?

You should be able to renew via mail by sending your old passport in. Check on their website. The Agency currently only by appt and it is near impossible to get one unless you are traveling urgently.
