Depending on where you are, visit the embassy website -NYC, D.C. or LA and go to the services tab. Under passport services go to lost passports. Follow the directions which include filling out the online form and downloading said form and mailing it. It will cost you$250, inumerable documents and lots of wasted time. You have to show up in person to have any issues addressed. they do not pick up calls. I was there a couple of weeks ago and got adviced to wait until they start issuing new ones.
I was there to get new one they advised I wait till August when they start issuing the new electronic ones. If you get one now you will have to renew again in August. Passport and id only on Tue and Thursday 9am to 1pm. All information on the website. Make sure you fill up application before you he there
