The form for SPA(Special Power of Attorney) can be found as .pdf on the Philippine Embassy Website(located on the right of the page under "Consular Forms", about 18 from the top) http://www.philippineembassy-usa.org/philippines-dc/consular-services-dc/faq-dc/#authentication The link above should take you to the list of Consular Services and the procedure for authentication of your SPA if needed under "Authentication and Acknowledgement" [Please note they only take cash or money order] However best thing you can do is call and ask prior to arrival. Also please be aware that there may be a time frame for this type of authentication and may require you to go back a second time. Hope this helps
