USA is on the list of countries entitled for tourist visa exemption. US passport holders may enter Thailand without a visa (tourism purpose only). Permitted period of stay is 30 days.
30 day tourist visa automatic upon entry. For longer you could visit a border city and reenter for another 30 days.

As of 04/06/2021, you still have to quarantine. If you travel after fully vaccinated (14 days after your last shot), the quarantine period is 7 days. You will no longer need fit-to-fly document but still need a negative COVID RT-PCR result. No you don't you get a 30day on arrival visa when you land in Thailand
You dont need it. But they recommend Hep A, Cholera and Yellow fever.
Do you mean with a valid US passport? Yes you can. You'll then get a visa on arrival upon entering the country, good for 30 days.
