Power of attorney drafted on a plain paper with two witnesses and notarised by a notary. If the witnesses are able to go along with you to the embassy you dont have to get it notarised. You also have to submit misc application form filled in with two passport sized photos. Proof of your status in U.S.A and proof of local address.With this you can get the P O A attested at the consulate after paying the required fees.
