You need to call medical records dept at RUSH. Call the main number and they will connect you to medical records dept. They will also tell you what all you need.

Low income and indigent people are allowed to get their medical records for free, as per Dave Jackson. This is a law in every State, though most hospitals sadly will not voluntarily let people know this.
