You will have to visit the consulate in person and verify your identy at which point you will be given a letter stating you in fact had a passport in thr past. The letter will be given to you in a sealed envelope which you will take to the your locla precint to file a police report. Once you have the police report you will return to the consulate with the police report and the other required documents to compltet the application and pay the fee of $190
