When I did the same thing years ago I had to take: 1) my birth certificate - certified copy. 2) marriage license also certified. 3) divorce decree or separation papers You can make an appointment ahead of time so you spent a long time waiting.
Birth certificate and proof of divorce or seperation dated. Must be stated in the divorce papers.
Government Issued Identification Card And/Or State Issued License, Social Security Card, And Piece Of Mail In Your Name That Isn't A Personal Letter
