Sonny, MountainView's Medical Records Dept. shouldn't have any difficulty finding your records so long as they still have them on site. Some medical facilities hold records 5 years, others 10 before microfiching (sp?) them to the archives where retrieving them is much more of a pain in the tailfeathers! However, they can track you by your last name or your Social Security number without your D.O.S. (Date of Service) fairly easily... sometimes easier than D.O.S.! Remember, you'll be required to pay for them...many of which are useless to us laypeople. It's kind of harsh to have to pay up to $1.50 per page for diagrams and graphs that no one but a specialist will understand so it's in your best financial interest to try to be as selective as possible because even the most minor procedure creates hundreds of pages of documents. Sadly, I had to buy them all to find out I actually only needed and could use less than half so be forewarned.
First step would be to contact that Hospital's Department and Inquire about all the records for that year.
