Do you pay up front or could you have them bill you?

I had to pay co-pays up front

I paid upfront. I am from Arizona.

If you are from another state, you have to pay up front. Then, i got a bill for additional charges because my insurance denied, even though the receptionist called and said it was approved.

Insurance paid.
