I just had the same type of problem, turns out it was Verizon that caused all of the issues. They were making unauthorized changes and settings on our account. Customer serviced blamed the store employees and vice versa. It took over 3 hours with the store manager on the phone with customer service to get it working, and they still were not sure how it was fixed.
I suggest visiting your nearest Verizon Store, or calling customer support from a different phone line if possible. Good luck!
