You can book Superstar shuttle service that picks you up from the airport and will take you to the resort and back to the airport. Cabana Bay does have shuttles to and from the parks everyday till 1am or 2am

No shuttle service is offered by Cabana Bay from hotel to airport. You can choose a cab or book alternate transport to the hotel. Contact Universal Concierge maybe they can help or recommend one. Their might be package that include transport but it depends on how you booked your hotel room.
No, the shuttle at Cabana Bay only takes you to a designated area, so you can get to the parks or CityWalk and takes you back to the hotel for free.
No. The shuttle can be booked when you are booking the hotel. The cost for the shuttle is very similar to the cab. That's why I took the cab instead.
If you have a hotel room, it usually is included but you have to check in at a table at the airport's shuttle exit.
