I lost my 2nd key card for my Model 3, so can I buy a new key card at the service center and program it by myself at home, since I have my 1st key card? Is the cost of the new key card - $12.50?

Yes, but they sell two cards at a time. I would just buy it from tesla website where you can buy just one.
