If you have a MyNYSTRS account, you can make the change online. Or you can complete and send NYSTRS a Member Name / Address Change form (GRE-50) and either send it to NYSTRS by U.S. mail at 10 Corporate Woods Dr., Albany, NY 12211, or by fax to (518) 447-4749.
