If your completing the form out for a change of address, no. You would need to pay the $32 for a new license card. They do have a small card that you can get to keep behind your DL that is dmv approved. IG Adroit000
U get a new card if u pay for one. Changing the address online wont get u a new card. You wud have to go into the DMV and request a chng of address and a new card for the amount of $35
