They will forward you one from the DMV automatically if your physician gives you permanent disability. If it is a temporary plaquard you need to complete the paperwork including a new physician statement
I had to reapply each time at the DMV to get new placards. The DMV mails you the forms the doctor needs to complete. It is not automatically renewed. Hope that helps
