There is no fee when leaving SF. There is a fee, based on number of axles, when you return to SF. They will mail you the bill.

Yes...a fee is charged for all vehicles travelling into San Francisco over the Golden Gate Bridge. I'm not sure what they charge for a bus though.
You pay a toll on the way back to SF, I dont know what it is. We were on a tour and thats what the guide told us.
