FMLA documents should be signed by the Patient's Doctor. This is what I learned, either it's for intermittent and continuous leave, make sure the signing Doctor should state "5 days in a week / 8 hours per day". That covers you for being absent any day of the weeek, or 5 continous day in a week.
Your FMLA paperwork is signed by your dr.
You can also check with your nurse coordinator for the right person to get it to.
When my dad underwent chemo/radiation, his oncology doctor signed my FMLA paperwork. If you're followed by multiple specialties, just start asking and they should help you figure out the appropriate doc to sign.
