Had a surgery at Harborview in 2002 or 2003 for my left hip, I recently reinjured it and current doctor said to go see whomever installed hardware. Whom do I call to schedule an appt, since I do not recall who I recieved services from?

I go to a different hospital, but I assume it's similar, when I call they ask for my name and date of birth that gives them the information they need to see who I have been seeing and other information. Try that 1st.

I agree with Adonis El-Mohtar, MFA. Also whatever hospital that performed your surgery will have your medical records, like who performed what & when, etc... They may also know what you need to do, who to talk to. They may also have the number for whom you need to be speaking with, regarding your insurance.
