They let you ask for it.
They usually offer you condiments from a orange tray as they bring you your order or put them in your bag automatically if you come through the drive through. But they do sometimes forget. Probably just a mistake of human nature, we should hope.
If you are in the drive thru just ask did you put ketchup in the bag. If you are inside they have always offered condiments from a orange tray.

That's just about everywhere you go now,
Usually customer request their chose - ketchup, mustard, ranch or etc. Every customer don't want ketchup sitting on top of their french fries when opening the bag.
Like the other two answers said. Probably a simple mistake. McDonald's will not give you anything unless you ask.
Just an oversight, I am sure. They usually give you an option of regular or spicy ketchup. Hint...take the spicy, it is good, lol.
