Wow, just wow...trying to get clients in for an assessment - local office is closed to the public due to Covid. When asked about services the employee simply kept repeating "we don't provide any services, we don't provide any services, etc" speaking over me and literally not listening. I hung up and tried back the next day when I was finally able to get a number to call for an assessment. When I tried to complain/explain the disastrous call the day before this woman laughed at me and hung up. What a lack of professionalism and customer service, I was appalled at my interactions with this office. Good luck

How come you don't answer your phone? Been trying for 2 days to talk to you and nothing. Tried to talk to a supervisor voicemail. Left a VM for a Supervisor, they are useless since they went to a team. Customer Service is a dying art. There is no such thing as a "secure line". I can't get through to anyone, how am I supposed to get my SNAP Benefits if no one will talk to me? I guess I won't be getting my SNAP Benefits. 09/03 Still no response, it must be nice to have food. 08/15/2020. Still incompetent and useless! 3-5 months to get my QMB and in the meantime $145 out of my account is being taken out! Incompetence

I've used the A & D for many yrs now and any problems I had were always handled with Kindness and respect. Nothing is worse that feeling vulnerable especially when you need help and are older. No complaints with their service.

Oregon ID go to DMV. Medicaid ID call your case manager. Medicare ID call Social Security.
Your U.S. birth certificate can be ordered online through several services, just type in Birth certificate in Google's search engine and click on the search icon. Additionally, you can go to the county of your birth' s website and order it. The site usually appear something like: anycounty.thestateyouwerebornin.gov. or type your county and birth state into Google search engine and it will take you to correct page. Beware of "free" sites, or "free" services from a non-official site, as they are usually "mining" REAL information with which to steal and identity from. There is a fee from most government entities, as they snail mail it, the paper is water marked to avoid fraud, and often notarized by the county clerk. It can take a couple of weeks. If you still live in/near your birthplace: Just take your state issued, or federal ID, current utility, rent receipt (something with your name on it, official type of mail), and usually 25 dollars or less for the fee. Carla Perkins i suggest for you to call seniors and people with disabilities. There are various programs they are through the state .speak to the receptionist.they will give you a number to call to initiate the process .you'll make an appointment for a worker to come out to evaluate what her needs are.that is the first step.they are rather timely about the initial appointment home call.two weeks to a month out.be patient its a long process but worth it I promise.they will explain the full program to you.good luck

Yes you have to make an appointment with her caseworker, and she will tell you what you need to do. You have to take some classes too.
Go to the Lincoln Building on SE 4th St. They will answer all of your questions. The cross streets are Pine & Oak Sts.

Implants are too expensive, they won't offer it.
You can now get them online, a social worker at Maybelle Center for Community helped me get mine awhile back. You could also ask if you could have it sent "general delivery" to your closest post office to pick up. I highly suggest seeing a case manager at Rose Haven, their number is in the little book made by street roots

Yes, you can go online on the DMV web site and fill out a chance of address form. If it applies to you, You will want to notify the social security administration of your address change also.
