Getting documents is a service that is only performed from 8-4. I totally get that it's hard to effectively communicate the details of the intricacies of a location like this, but there is an overuse of "8-5" hours being posted on websites with no caveat. Again, I get why this might be the case, but it needs to be communicated more effectively. I would give the benefit of the doubt and give 3 stars, but there's an insult to working people to remove another hour of window time to complete errands that involve this service. If banking hours were insulting enough, this takes it another step lower. I sympathize with the logistics higher ups face with running a health department and by no means intend to insult those who just happen to be in the trenches, but perhaps this will save someone a trip across town and schedule coordination efforts. Best to call ahead to get explicit outlines of variable hours.

I don't think there's a certain set day for immunizations. It's just walk in. You go in early and sign in, then wait for them to tell you what time they'll be able to see you

For immunizations,std,and birth control services. Unfortunately general health concerns cannot be addressed at this facility.
And just to clarify, that is $15 per individual person's birth certificate. For instance, I got certificates for five children, $75 with cash or check. $17 each with debit card.
Call back to the health department in the county in which you were born. At least I applied and received mine that way.
I don't believe so, but please call to verify. If you have All Kids then there are pediatric dentists who accept that in the area.

Birth dates of individuals whom you are applyining for, your legal I.D. and reason you are applying. You must fill out an application for it. $15 cash or check each certificate. Add $2 if using debit card.
You should call and ask, because my understanding is that you cannot visit unless you have Medicaid. I do know that they work by appointment only, and they only do TB testing for state and federal employees now.
You need to call them personally and ask. I do not know if they will mail you a copy, or if they require you personally to come get it. I've heard procedures vary from state to state.
