I am working out of state and last year, September 2021, my mother moved to this facility against family advisory. Recently, my mother's air conditioner went out and nothing was done to service it that day which was a hot July day. Her oxygen tank broke and she was not able to get any assistance from any nursing staff or any type of employee. Luckily, she was able to find a number and spent all day trying to find someone to help her. She even had the EMTs to come out and they told her they could take her to hospital. In June, my mothers account was overdrawn. They not only charged her twice but both withdrawals were before her social security was deposited June 2nd and 4th. The worst part is she has had 3 encounters with bed bugs and several times she has not received her medications. She was hospitalized and no one was aware she was gone. Upon returning, her discharge medications were given to the nursing staff. They lost her prescription. It was over a week before it was located, taken to pharmacy to be refilled, and provided to her to begin treatment. I understand staff shortage but certain patients need there medication and if the staff is not able to consistently disseminate medication than the able patients should be allowed to keep up with their own medications. Now they are switching out nurses and training staff to become med aids. They are rarely checking, if at all vitals, which is important to establish parameters before giving blood pressure medications. When I came into town to visit my mother, I had to remove her because of the bug infestation. I was ensured by the DON this would be resolved. Yet, here we are again. This place is not sanitary. If a person has bedbugs why would the employee who cleans transport it all over the facility by continuing to vacuum from room to room? I had just purchased my mother a new queen size mattress because she was provided a small twin. She was falling out of the bed and hurting herself. The new mattress is now infested with bedbugs. The administrator made excuse but did not take swift action. My mom was left in the room and all they did was spray with the promise to finally switch her to another room in 2-3 days. This is unacceptable! This should have been resolved the first time. My mom stays in her room so she didn't pick up from somewhere. The admin, Mark asked my mom if she had dried her clothes at a certain dryer because someone had said they saw bedbugs in it. This whole place is infested in multiple areas if not entirely. I advised them the last time they would need to treat the whole hallways in order to resolve this issue. I was told they would switching patients to another area in order to treat their rooms. No accommodations were made so I took my mother out of the facility. Just imagine residents who are not able to speak up. They allow residents to sit out front of entrance to smoke. Some residents are smoking inside which is evident by the smell of smoke transferring through vents. My mother has COPD and uses supplemental oxygen. The hallway carpets are past needing shampooing but recarpeting. The residents are bringing in alcohol and/or drinking at the outside entrance. While walking my mom out, a resident was in a hurry to get by, and minutes later I saw him at the entrance gate meeting up in the dark with a shady character on a bike. This was no food, Walmart, or Amazon delivery, Obviously, it was something illegal. I will be contacting health and human services. I will be working diligently to remove my mother from this facility. My saving grace has been Ms. Cat and a few other techs. PLEASE DO NOT MOVE YOUR LOVED ONE TO THIS FACILITY!!!

My aunt in 505 room is infested with bed bugs and had to stay the weekend with her daughter. She was told that only her room would be bombed Monday. She advised us that the room has been infested since day one of her stay there. My concern is how will one treatment resolve this issue? Why isn't the entire facility getting treated? Secondly, the carpet is filthy, worn, and smells in the hallway. This will be reported to the health department on Monday. Best regards, Shalanda Williams

I am very concerned about the screening process of some of the residents.

Each room is private and can be occupied by a single person. Our age requirement is 55 or older. Our private pay rate is $2295. However if they are on Medicaid the rent is based according to their social security check with a minimum of $771. Please let me know if you have further questions.
