This particular location seems to be famous for creating their own work hours and canceling passport appointments upon arrival. I had a pleasure of witnessing this unprofessional chaos for myself this morning, while waiting with a group of people outside in front of closed doors. Hours listed 9am-5pm, with doors opening at 11:18 am by an employee who seemed to strongly dislike Mondays. She did however supplied me with a phone number that I could call to reschedule. I’ve been calling ever since, on redial since 11:30 this morning. It is 5 pm now, so I guess I should start again tomorrow.

1. Fill out Form DS-11 2. Provide evidence of citizenship, i.e bring a copy Certificate of Birth. Original copy is required. 3. Bring your SS 4. For anybody aged 16 and above bring your ID. For anybody aged below 16, orig. ID of both parents (and parents signature) are required. Signing in person (must be present). Make a photocopy of your ID. 5. Make 2 passport size photographs on site (for $15) or bring them with you 6. Pay the required fees. For adult it is $110 application fee + $35 execution fee. For children aged below 16: it is $80 + $35. You can also make a Passport Card if you add $30 on top of the $110 for adult, making it a total of $140 + $35. For children's card, add $15 making it a total of $95 + $35 And if you are in a rush (must leave in a month or less) bring the original overseas airplane ticket(s) and pay an extra $60 for expedite cost.You can download (and print) Form DS-11. If you come from Mexico or Canada, yes you must have your passport with you; but if you leave from Philadelphia you will only need your valid photo ID or: the "Real ID" if you already have one
They usually send the passport and the birth certificate back in different envelopes. The BC could arrive a few days later. But if you have not received the BC yet, you may want to check it with the issuer
