Primary One/Columbus Health Department Dental Clinic is a sliding fee scale clinic based on household income. Visits can be as low as $40.00 for an extraction. Walk in emergency time for toothaches are Monday thru Friday starting at 7:30am. Bring picture ID and proof of income, pay stub, ss award letter, unemployment info, etc. Proof of residency also for correct address, utility bill etc. To become a patient for ongoing treatment, bring proof of income, picture ID, proof of residency, fill out paperwork and you will get an appointment to see the dentist for an assessment, xrays and a consultation. 240 Parsons Ave 43215. It is best to make an appointment but it is possible to be seen same day I would get there early. The sexual health clinic is on the 2nd floor and womens health is on the 1st floor.

I dont believe Columbus public health provides out of state birth certificates. You will likely need to contal vital statics in that state or order online.

All services at the TB clinic and most things at CPH are offered on a sliding scale. Nobody is turned away based on their ability to pay
Hiya! The sexual health dept @ CPH is open from 8 am - 4:15 pm. I would recommend getting there a bit before 8 if you can, because they fill up fast with it being first come, first serve. I actually work in sexual health prevention. If you have other questions, please feel free to ask!
