Primary One/Columbus Health Department Dental Clinic is a sliding fee scale clinic based on household income. Visits can be as low as $40.00 for an extraction. Walk in emergency time for toothaches are Monday thru Friday starting at 7:30am. Bring picture ID and proof of income, pay stub, ss award letter, unemployment info, etc. Proof of residency also for correct address, utility bill etc. To become a patient for ongoing treatment, bring proof of income, picture ID, proof of residency, fill out paperwork and you will get an appointment to see the dentist for an assessment, xrays and a consultation. 240 Parsons Ave 43215.
